General Practice Pharmacist

A dynamic general practice pharmacist driving safer prescribing, high-quality medication reviews and long-term condition management. You'll optimise polypharmacy, streamline repeat/acute workflows, strengthen links with community and hospital pharmacy, and boost patient outcomes across a rural, older population.

Main Duties Of The Job

Patient-Facing Work Deliver long-term condition clinics (COPD, asthma) with a focus on medicines optimisation. Complete clinical medication reviews for practice, care home and domiciliary patients, providing recommendations for nurses and GPs. Offer patient-facing and telephone support for medicines queries and adherence. Attend MDTs and contribute to ad-hoc project work.

Medicines Management Respond to medicines queries from staff, community pharmacy and patients. Review medicines linked to unplanned admissions and implement changes for high-risk groups. Reconcile post-discharge medicines, resolve discrepancies and ensure continuity of supply. Signpost patients appropriately for clinical issues, results, minor ailments and LTC reviews.

Prescribing & Safety Manage repeat prescribing, reauthorisation, monitoring and acute queries. Use risk stratification searches to identify high-risk cohorts. Support development of services with medicinal components. Analyse prescribing data, undertake audits, implement MHRA alerts, product withdrawals and guideline updates. Monitor prescribing against RAG lists and support formulary and NICE compliance.

Education, Quality & Collaboration Train the primary care team, support CQC compliance and contribute to public health campaigns. Work effectively with practice, locality, community and hospital pharmacy teams, maintaining strong cross-service links.
