Permanent Supportive Housing Case Manager - Arlington Mill

Position Summary:

The Permanent Supportive Housing Case Manager is a full-time position which reports to the Vice President, Housing. The Permanent Supportive Housing Case Manager is responsible for assisting persons eligible for PathForward’s Permanent Supportive Housing (PSH) programs to obtain and maintain stable housing.   Since the contract in place mandates direct service to clients, this position is required to report to the office 40 hours a week.   The position will be primarily located at the Arlington Mill Residences, an eight-unit supportive studio program primarily serving formerly homeless individuals with disabilities. The position shall be responsible for maintaining a safe, healthy and welcoming living environment for tenants. This position will work in collaboration with a team of life skills coach who will be providing 60 hours per week of on-site support to tenants.

 

Key Responsibilities

• Assist in engaging eligible program participants when there is a vacancy.
• Assist prospective program participants in filling out the intake referral applications.
  1. Obtaining any other supporting documentation needed (i.e. ID, Social security card, etc.)
• Coordinate with the Housing Locator to identify available affordable units.
  1. Request housing locations and inspections annually as per program requirement.
• Assist prospective program participants in securing and filling out leasing applications.
• Assist program participants with moving into their unit once approved.
• Schedule home visits once a week with program participants, with a focus on successful tenancy.
• Coordinate appropriate linkages to available County and other mainstream services (i.e., mental health services, addiction recovery programs, medical appointments and employment resources)
• Provide transportation to and from appointments as needed.
• Maintain accurate and complete weekly case notes for all program participants assigned to caseload.
  1. Maintain participants’ files including release of information, up to date case notes, weekly service plans, and individual service plans.
  2. Complete rental calculations annually or as needed.
  3. Submit rental invoices at the beginning of each month and/or when needed.
• Input accurate and complete data timely (on a daily, weekly, monthly basis) in the Efforts to Outcomes HMIS client database.
• Complete bi-annual assessments and goals set out in the Individual Service Plan.
• Assist clients with their Activities of Daily Living skills.
• Report on program participant successes and challenges during bi-weekly Supportive Housing Team Meetings.
• Act as on-call contact person for property management companies/landlords providing units to program participants to mitigate possible tenant lease violations.
• Attend landlord liaison meetings as scheduled (Monthly and/or quarterly).
• Coordinate with PathForward’s Development team to maximize use of in-kind donations and volunteers in the PSH program.
• Communicate effectively with other service providers, conduct treatment team meetings as needed to provide the best continuity of care for program participants.
• Work as part of a team, meeting regularly to share information, strategies, mutual support, and ideas with the primary goal to address the long-term needs of homeless individuals including access to and maintaining permanent housing.
• May be required to work at an offsite location to provide Case Management services,
  1. Be available as a resource person at that site.
  2. Staff the offsite office and provide case management to the 8 tenants on the property.
  3. Liaise with the site’s Residential Services Coordinator, leasing staff, DHS, and supervisor. 

 

• Adhere to N.A.S.W. Code of Ethics.
• Always maintain participants confidentiality
• Update skills by taking advantage of available training opportunities.
• Attend any other meetings as scheduled.
• Meet with supervisor bi-weekly to discuss caseload and any client issues. 
• Any other duties as assigned including crisis intervention.
• All other related program duties and projects as assigned including attending all mandatory staff events and trainings: Relias, Mandt training, and other applicable trainings.

Qualifications and Skills

• Must be a self-starter.
• Excellent written and oral communication skills.
• Excellent interpersonal and organizational skills.
• Excellent computer skills and experience with outlook and Microsoft office.
• Demonstrated ability to work well with individuals in crisis.
• Fundamental characteristics including empathy, patience, and persistence are required. 
• Must have a valid driver’s license and clean driving record.
• Bi-lingual (Spanish/English) a plus

 

Experience and Education Requirements 

• minimum of a BSW or Bachelor’s degree in a related field. 
• Experience with substance abuse, mental health issues and chronically homeless individuals is preferred.
• Experience working with youths who are experiencing homelessness.

 

Physical Requirements:

• Can be prolonged periods of sitting at a desk and working on a computer.
• Must be able to drive/walk to different properties and/or climb flights of stairs if needed for home visits. 
• Works primarily in a sedentary environment, utilizing various means of technology.
• Occasionally required to lift up to 25 lbs.

Monday through Friday, 9am - 5pm.
