POSITION SUMMARY 

The PSH Case Manager Supervisor oversees tenant-based case management services to support people who have experienced homelessness in securing and maintaining permanent housing. This work is completed within Jaydot's Permanent Supportive Housing (PSH) Program, funded by a contract with the DC Department of Human Services (DHS). The PSH Case Manager Supervisor directly manages tenant-based case management staff that Jaydot employs under the PSH Program, all with the goal of ensuring the housing stability and well-being of clients exiting homelessness. 

ESSENTIAL RESPONSIBILITIES

• Manage and provide clinical oversight for up to seven (7) tenant-based PSH Case Managers 
• Monitor and track case management staff engagement with participants
• Review case notes through the lens of quality of information captured and accuracy
• Review, evaluate, and approve participant case plans for quality and effectiveness
• Meet individually with Case Managers to plan and review cases, discuss engagement strategies and evaluate the effectiveness of the Case Management staff and services
• Provide guidance for case managers on addressing particularly challenging participant situations and assist with connections to more intensive support services, as needed
• Review and approve all notices issued to participants for accuracy and completion
• Analyze and ensure Housing Stability Plans are updated quarterly and track participant's progress toward achieving their goals
• Ensure that quarterly re-certifications are completed and designated software and files are up to date with this information
• Ensure all case management documentation is completed in compliance with contract standards
• Engage in case conferencing with District government staff assigned to discuss cases that may need to include but are not limited to more intensive case management or transfers
• Participate in monthly District/Provider meetings and facilitate internal staff meetings as requested by the Director of Programs

• Attention to detail: The PSH Case Manager Supervisor aims to leave things better than they found them. As the lead on case management documentation they notice and fix errors that others might overlook. When mistakes happen (which they will!), they address them quickly and look for ways to prevent similar errors.

REQUIRED EDUCATION AND EXPERIENCE

• Any one of the following may meet the education requirement for this role. Applicants who are not an LICSW or LPC will require District government approval.
  • Licensed Independent Clinical Social Worker (LICSW) or a Licensed Professional Counselor (LPC) in the District of Columbia, OR
  • Licensed Graduate Social Worker (LGSW) and/or Licensed Graduate Professional Counselor (LGPC), OR
  • Master's Degree in Social Work, Mental Health Counseling, or related Human Services field from an accredited College or University, OR
  • Bachelor's Degree in the Human Services field from an accredited College or University and four (4) years of work experience providing Permanent Supportive Housing Services to individuals/families who have experienced homelessness
• Minimum two (2) years of experience in case management, mental health services, and direct service delivery to individuals and families experiencing homelessness and/or other marginalized populations required
• Minimum five (5) years directly supervising frontline staff required
• Professional knowledge of the theories, principles, techniques, and practices of social service delivery systems required
