PRN Mental Health Case Manager — Discharge & Care Planning

Dormont Manufacturing Co

Dormont Manufacturing Co is seeking a Case Manager to handle clients with mental illness or substance abuse issues. You will assess needs, create service plans, and work with community resources for client support. A Bachelor’s degree in a relevant field is preferred, along with experience in healthcare. This role requires strong communication skills, crisis intervention abilities, and a commitment to enhancing patient care. Benefits include a 401K retirement plan.
