Mental Health Case Manager: Discharge & Care
SAN-Antonio-Behavioral-Healthcare-Hospital-
SAN Antonio Behavioral Healthcare Hospital is seeking a Case Manager responsible for managing a caseload of clients with mental illnesses or substance abuse. Key duties include assessing client needs, developing service plans, and coordinating care with community resources. The ideal candidate has experience in healthcare case management and must possess a valid CPR certification. Bi-lingual candidates are preferred.
