Bilingual Pediatric Community Health Advocate (Part-Time)

Humboldt Park Health in Chicago is seeking a Bilingual Community Health Worker (Part Time) to support families via a pediatric van program. You will educate families, perform home visits, and coordinate with providers to improve healthcare access on-site in Chicago. The role requires completion of a CHW program within 1 year, bilingual English/Spanish, a high school or equivalent diploma, and BLS certification.
