Responsibilities

• Assess patient and family needs; develop and implement an individualized plan of care in collaboration with the interdisciplinary team; monitor progress and adjust as needed.
• Deliver direct nursing care, administer medications, and respond to changes in patient condition with clinical judgment and compassion.
• Educate patients and families; provide discharge planning and self-care guidance to support health beyond hospitalization.
• Coordinate with physicians and other members of the healthcare team to ensure care is aligned with patient goals and updated as conditions evolve.
• Promote infection prevention, safety, and a healing environment; document education and patient responses to care.

Requirements

• Active New York State RN licensure
• BLS certification (AHA)
• ACLS certification (AHA)
• NIHSS certification
• Minimum 2 years’ RN experience in a Med/Surg environment
• Ability to work in a Short Term Acute Care Hospital setting with day/night variable shifts; willingness to participate in on-call shifts (2 on-call shifts in a 4-week period)
