Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year in Adult Home Health Experience or 1 full year in Hospice Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficient in HomeCare HomeBase - Proficient in OASIS - Must be willing to travel up to 75 miles Description: Provides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required. Periodic office meetings will take place. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day equating to roughly 6-8 productivity points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Previous experience in home health or hospice is required. First-timers are not accepted; recent specialty experience is necessary. Weekend requirements will vary. On-call requirements also vary. Documentation must be completed within 24 hours of the patient appointment. Delayed and incomplete documentation that impacts billing and follow-up patient care may result in cancellation of the contract. There are no guaranteed hours with this contract. Clinician must achieve weekly productivity points. If enough patients are not scheduled each week, floating to another nearby clinic is an option. Local applicants are accepted under the same terms. All travelers must be proficient in HomeCare HomeBase EMR. All documentation must be within the last 6 months, or a retraining course may be required.
