Responsibilities:

Collaborate with our interdisciplinary team to assess, develop, and implement individualized care plans for residents
Provide clinical expertise and guidance to our professional staff, ensuring the delivery of evidence-based care
Conduct patient assessments, develop and revise care plans, and evaluate patient progress
Participate in quality improvement initiatives to enhance care processes and outcomes
Foster strong relationships with residents, families, and healthcare professionals to ensure exceptional care
Maintain accurate and detailed records of patient care and treatment
