Job Posting: Hospice MSW PRNVisiting Nurses Association (VNA) Position Type: PRN (As Needed) Schedule: Scheduled days per week - TBD Minimum Availability Requirement: Must be able to work a minimum of 10 hours every 90 daysPosition SummaryVisiting Nurses Association (VNA) is seeking a compassionate and experienced PRN Medical Social Worker (MSW) to provide psychosocial assessments, counseling, and resource coordination to hospice patients and their families. This role focuses on supporting patients through end-of-life care by addressing emotional, social, and practical needs in accordance with physician orders, hospice plans of care, and regulatory requirements.Key ResponsibilitiesConduct comprehensive psychosocial assessments for hospice patients and familiesProvide emotional support, counseling, and crisis intervention related to end-of-life careAssist patients and families with advance care planning, community resources, and bereavement supportCollaborate with nurses, physicians, chaplains, and the interdisciplinary hospice team (IDT)Participate in care planning meetings and case conferences as requiredEnsure compliance with hospice, Medicare, state, and agency regulationsComplete documentation accurately and within required timeframesParticipate in required compliance training and departmental development activitiesQualificationsMaster 's degree in Social Work (MSW) from an accredited programCurrent, unrestricted social work licensure in the State of Kansas (LSCSW, LMSW, or LCSW, as applicable)Minimum of one (1) year of hospice, home health, or healthcare experience preferredKnowledge of hospice philosophy, end-of-life care, and community resourcesStrong communication, documentation, and organizational skillsAbility to work independently while collaborating with an interdisciplinary teamValid driver's license, reliable transportation, and auto insuranceVisiting Nurses Association (VNA) is an Equal Opportunity Employer.Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.
