Pharmacist II Clinical Pharmacist

Location: Utica, NY

This Clinical Pharmacist role offers the opportunity to practice in a decentralized hospital environment, supporting critical care areas such as the emergency department and intensive care unit. Reporting to the Pharmacy Operations Manager, Clinical Pharmacy Manager, or designee, the pharmacist will play a vital role in optimizing patient outcomes through direct collaboration with multidisciplinary teams.

Key Responsibilities:

• Deliver clinical pharmacy services within assigned hospital units, ensuring safe and effective medication use.
• Independently review and verify medication orders for dosing accuracy, contraindications, interactions, and therapeutic appropriateness.
• Participate in multidisciplinary rounds, contributing expert recommendations on medication therapy management.
• Conduct medication reconciliation, patient counseling, and respond to drug information inquiries.
• Monitor patient medication profiles to identify and resolve drug-related issues.
• Provide education, precepting, and mentorship to pharmacy residents and students, supporting the PGY1 residency program.
• Prepare sterile IV admixtures and compounded sterile products following USP standards and facility protocols.
• Oversee and train pharmacy technicians involved in sterile compounding activities.
• Engage in medication safety initiatives, quality improvement projects, and help develop departmental policies.
• Ensure compliance with New York State and federal pharmacy regulations.

Qualifications:

• Doctor of Pharmacy (PharmD) from an accredited institution.
• Active New York State pharmacist license in good standing.
• 13 years of hospital pharmacy experience with clinical expertise in settings such as ICU or emergency departments.
• Completion of a PGY1 pharmacy residency.
• Board certification by the Board of Pharmacy Specialties (BPS) preferred.
• Demonstrated interest and experience in teaching and precepting pharmacy residents or students.
• Strong clinical knowledge of pharmacotherapy and medication management principles.
