Clinical Pharmacist II

A Clinical Pharmacist II position is available in Utica, NY, offering an opportunity to work in a dynamic inpatient hospital setting. This direct hire role entails providing advanced clinical pharmacy services in specialized units such as the emergency department or intensive care unit, supporting optimal patient care and collaborating with multidisciplinary healthcare teams.

Responsibilities:

• Deliver clinical pharmacy services in assigned hospital units, focusing on patient-centered care.
• Independently review and verify medication orders, ensuring proper dosing, route, frequency, and safety.
• Participate in multidisciplinary rounds, offering evidence-based medication therapy recommendations.
• Conduct medication reconciliation, patient counseling, and respond to drug information inquiries.
• Monitor medication profiles, identify drug-related problems, and suggest appropriate interventions.
• Support pharmacy residency programs by precepting residents, providing educational presentations, and evaluating performance.
• Prepare sterile IV admixtures and compounded products compliant with USP standards and protocols.
• Train and supervise pharmacy technicians and staff in sterile compounding procedures.
• Mentor pharmacy students and contribute to their clinical education.
• Engage in medication safety initiatives, quality improvement, and policy development.
• Ensure compliance with New York State and federal pharmacy regulations.

Qualifications:

• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active and good standing New York State pharmacist license.
• 1 to 3 years of hospital pharmacy experience.
• Clinical pharmacy expertise in acute care units such as emergency or intensive care.
• Strong knowledge of pharmacotherapy and medication management.
• Completion of a PGY1 pharmacy residency.
• Board certification from the Board of Pharmacy Specialties (BPS) preferred.
• Experience in precepting pharmacy residents or students is highly valued.
• Passion for clinical education and mentorship.
