We are looking for a compassionate and resourceful Case Manager to support residents in San Francisco, California through a Contract position. This role is ideal for someone with strong computer and customer service abilities who can balance administrative responsibilities with in-person support in the community. The position focuses on helping formerly homeless adults maintain housing stability, access essential services, and improve overall quality of life through consistent engagement and advocacy.

Responsibilities:
• Oversee a high-volume portfolio of resident cases, providing individualized support plans tailored to housing stability, wellness, and daily living needs.
• Meet with residents regularly through room visits, outreach efforts, and follow-up appointments to assess progress and identify emerging concerns.
• Deliver ongoing case management services, including needs assessments, service coordination, and practical guidance aligned with each resident’s goals.
• Help residents secure and maintain public benefits, stay current on rent obligations, and address barriers that may affect successful tenancy.
• Partner with property staff during unit walkthroughs, pest control visits, and habitability reviews to address living condition issues promptly.
• Connect residents with community resources such as healthcare, behavioral health, recovery support, and employment programs, and encourage successful follow-through.
• Foster a supportive tenant environment by organizing community activities, group engagement opportunities, and resident-centered events.
• Respond calmly to urgent tenant situations by using de-escalation techniques, documenting concerns, and coordinating with internal teams or external providers when needed.
• Maintain accurate case files, service records, and confidential documentation while attending required meetings and reporting concerns in a timely manner.• Demonstrated experience in case management, social services, housing support, or a related administrative support environment.
• Strong computer proficiency, including the ability to manage records, documentation, and service-related data accurately.
• Excellent customer service and communication skills with the ability to build trust across diverse populations.
• Comfort working in the field and traveling on foot within the Tenderloin area for resident support tasks such as outreach and mail runs.
• Ability to manage multiple priorities and a large caseload while maintaining organization and attention to detail.
• Experience supporting individuals facing mental health, substance use, medical, or housing-related challenges is highly valued.
• Sound judgment in crisis response, including the ability to remain composed and use effective de-escalation strategies.
• Commitment to confidentiality, professionalism, and collaborative work with staff, property teams, and outside service providers.
