Screen for interactions, duplications, and contraindications. Resolve issues with prescribers and document interventions.Preparation & Dispensing: Accurately compound specialized sterile/non-sterile medications (e.g., chemo, TPN) per regulatory standards. Ensure correct dispensing, labeling, and inventory control.Clinical Expertise & Education: Provide expert pharmacotherapy information to providers. Offer comprehensive, patient-centered counseling on complex medications (action, administration, side effects, adherence).Collaboration & Care Coordination: Participate in rounds and committees. Develop patient-specific medication plans. Perform medication reconciliation upon admission/discharge to prevent errors and ensure safe transitions of care.Quality & Development: Supervise technicians, ensuring quality control and regulatory compliance. Mentor students/residents.
