Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with individual's functional level and potential. • Bachelor's Degree from an accredited Occupational Therapy Program, or
• Master's Degree from an accredited Occupational Therapy Program, or
• Ph.D. from an accredited Occupational Therapy Program, or equivalent

educ/experience * Coursework/Training: Occasional specialized Occupational Therapy training to meet a particular department need, or equivalent educ/experience

• Upon hire: Oregon Occupational Therapist License
• Upon hire: National Provider BLS - American Heart Association
