Job Title Center Medical Director Company Oak Street Health Role Description The Center Medical Director at Oak Street Health leads the clinical team, ensuring high quality patient care and a positive workplace culture. The role combines 80% clinical practice with 20% administrative responsibilities, and includes mentorship, strategic implementation, and leadership across the organization. Core Responsibilities Support Organizational Strategy: Collaborate with Practice Manager and Outreach Manager/Director to achieve objectives such as Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work, and DE&I leadership. Drive implementation of OSH strategic initiatives and follow regional leadership directions. Communicate organizational information to care teams and elevate concerns and ideas upward. Provider Management: Coordinate onboarding of new providers. Model clinical excellence and support care model implementation. Ensure evidence‑based care that keeps patients healthy and out of the hospital. Teach and implement Oak Street clinical guidelines. Support preventative care and value‑based risk model adoption. Address root causes when center metrics are not met. Leadership and Coaching: Create a coaching culture for growth of providers and teams. Address feedback and concerns constructively. Conduct performance reviews and goal planning. Culture: Celebrate team successes and identify improvement opportunities. Achieve triad best practices and maintain a positive, joyful practice environment. Other duties as assigned. Qualifications M.D., D.O., A.P.N., or P.A. graduates. Board certification in Internal Medicine, Family Medicine (for M.D./D.O.), or Family, Adult, or Gerontology (for A.P.N.). Experience in clinical leadership, coaching providers, and metrics‑driven quality projects. Passion for fast‑growing organizations and commitment to older adults and low‑income patients. Strong leadership skills, problem‑solving ability, and calm under pressure. 5+ years in outpatient practice; fellowship or advanced degrees welcomed. US work authorization.
