Qualifications:

• Valid RN license (compact license accepted)
• At least one year of recent experience in Stepdown, Hospice, or Telemetry settings
• CPR/BLS certification required; ACLS preferred
• Strong communication skills and ability to collaborate with interdisciplinary teams

Key Responsibilities:

• Deliver compassionate and evidence-based care to Stepdown, Hospice, and Telemetry patients
• Monitor patient status, report any clinical changes promptly, and adjust care plans as needed
• Document assessments, interventions, and responses accurately
• Educate patients and families on condition management and post-discharge instructions
• Coordinate care with physicians, pharmacists, and allied health professionals
