Overview Physical Therapist Physical Therapist Job Summary We are seeking a dedicated Physical Therapist (PT) to provide exceptional, patient-centered care. In this role, you will manage the therapeutic care process for an assigned group of patients, collaborating closely with physicians and the patient care team. You will conduct comprehensive patient assessments and contribute to the development and implementation of individualized care plans. Your focus will be on achieving patient goals, fostering patient engagement, and ensuring a positive patient experience. The Physical Therapist will deliver or oversee the majority of the patient's physical therapy interventions, supporting patients throughout their continuum of care. Key Responsibilities Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics. Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care. Provide purposeful and skilled therapeutic care, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care. Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions. Prioritize caseload, determine daily staffing needs, and coordinate assignments/handoffs. Required Qualifications Bachelor's Degree in Physical Therapy. Licensed Physical Therapist. Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of employment start date. Minimum of 2 years of experience in an acute care setting. Preferred Qualifications Not specified. Skills & Competencies Proficiency in patient assessment and evaluation. Experience in developing and implementing individualized care plans. Knowledge of therapeutic procedures and techniques. Ability to instruct patients and caregivers in the use of therapies and assistive devices. Strong prioritization and organizational skills.
