Registered Nurse - Transitional Coordinator

$44.56 per hour

As a Transition Coordinator, you’ll support patients and families as they navigate important care decisions, drawing on information from multiple sources to help them move safely and confidently to the right place of care. Description As a Registered Nurse (RN) you will provide a wide variety of nursing services to patients, families, communities and populations, while ensuring their safety and well‑being. You will utilize nursing processes—critical thinking, problem solving and decision making—to teach, counsel and advocate on behalf of patients and families. Your role includes a leadership element in supporting an integrated and holistic approach to patient care, health promotion and maintenance, and you will deliver safe, quality, patient‑ and family‑centered care that reflects the shared vision and values of the organization. Qualifications Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP) certification. If you have not practiced as an RN within the past five (5) years, completion of the following free courses prior to employment: CRNA Medication Management (Chapters 1‑4) at and NextGenU Health Assessment Resources Certification at . Additional Required Qualifications Five or more years’ experience in good standing with CRNA. Experience performing comprehensive assessments of individuals within their physical and social environments. Skilled in collaborative practices and concurrent care planning. Experience with Patient and Family Center Care (PFCC) and Complex Discharge Management. Solution‑focused and able to navigate difficult conversations. Ability to operate through high levels of stress, fluctuating priorities, and time‑sensitive deadlines. Physical competence: tolerate long periods of sitting or standing, perform repetitive movements, lift and carry 10‑15 lbs, walk distances up to 3 km per day, and provide hands‑on care during assessment (transfers, bariatric clients, bending, twisting, walking, intervening for client safety). Preferred Qualifications Baccalaureate degree in nursing (RN). Two or more years’ experience within Home and Community Care (Case Management). Trained RAI HC and interRAI Contact Assessment. Experience in a CCH (Type A, B, or C) environment. Training in specialty geriatric or palliative care.
