Nurse Practitioner or Physician Assistant

Become a part of our caring community Humana's Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner (NP), or Physician Assistant (PA) to join our team at our clinic in Jacksonville, FL As a Nurse Practitioner (NP) or a Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more-all working together to deliver personalized, high-quality care. You will report to the Associate Medical Director, and work at our clinic located at Phillips Hwy, FL Team-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness. More Time with Patients: See fewer patients per day and spend more time delivering meaningful care. We foster a welcoming, inclusive environment where teamwork and growth are prioritized. You will provide comprehensive care to seniors following clinical standards. You will collaborate daily with your care team to ensure coordinated, high-quality care. You will manage referrals, hospital/SNF coordination, DME, and home health services. You will document care with support from quality-based coders. You will participate in on-call rotation and support growth plans at your center. You will have experience with providing value-based care. You will work on-site. Master's degree in nursing with board certification. Experience in value-based care, primary care or Medicare Advantage settings. Familiarity with HEDIS quality indicators and Medicare guidelines.
