Habitat Health seeks a Functional Wellness and Rehabilitation Educator to lead onboarding and training for PTs, OTs, Activities Coordinators, and PCAs at PACE Centers. This field-based role oversees learning fidelity across centers and drives standardization of workflows and competency assessments. The incumbent will partner with Practice Operations, Center Leadership, and People Team to optimize materials, documentation, and processes while traveling regionally to support new center launches in #J-18808-Ljbffr Habitat Health
