Assistant Director of Assessment Schedule: Full-time Triangle Springs is part of Lifepoint Health, a diversified healthcare delivery network. How you'll contribute Direct and perform screening of potential patients for admission into the program and initiate the integrated assessment process. Assist administration, physicians, and clinical staff in assessing pending admits to determine appropriateness of level of care and communicate pending admissions to the nursing department. Use approved admission criteria and make recommendations concerning the level of care for treatment using the least restrictive level appropriate; initiate the plan for episode of care at admission to all levels of care. Function as a liaison between physicians and families, coordinate with transportation services and other mental health facilities to organize admission procedures. Ensure all vital patient assessment information is referred accurately to attending physicians and that patient historical information obtained is complete to facilitate accurate determination of appropriate level of care. Communicate with external reviewers and referral sources, conduct all required external reviews and maintain documentation of all such interaction while following HIPAA guidelines. Ensure third‑party payors are notified of or participate in decisions about appropriate transitions between levels of care. Prepare morning meeting reports to communicate pertinent census information, referral sources, and pending admissions. Consistently provide services to current referral sources to assure their satisfaction and continued association. Assess, coach, and develop teams' skills, and hold teams accountable to high standards. Be knowledgeable in CMS, JCAHO, EMTALA, and State regulations and standards of performance; regularly audit functions to ensure compliance. What we're looking for Education : Master’s degree in social work, counseling, or related field required or licensed degree per state practice guideline. RN licensure may be used per state of practice guidelines. Experience : Previous management experience in a psychiatric health care facility; direct experience working in assessment for chemical dependence, dual diagnosis, psychiatric, and geriatric patients. Must have strong clinical assessment skills. License : Current unencumbered clinical social work, counselor, or RN license.
