What You Will Do

• Provide meaningful instruction to individuals to develop and strengthen community living and independent living skills.

• Assist individuals with activities of daily living, personal care, and health-related needs as appropriate.

• Promote community inclusion by supporting individuals in accessing and participating in community activities.

• Support the development of positive relationships, social skills, communication skills, and leisure interests.

• Help individuals identify and access appropriate community services, resources, and opportunities.

• Facilitate the development of independent community travel skills.

• Meet with individuals to discuss behavioral concerns, challenges, and sensitive topics in an appropriate and respectful manner.

• Encourage individuals to identify potential outcomes, make informed choices, and develop problem-solving skills.

• Assist individuals in communicating effectively with Care Coordinators, vendors, healthcare providers, landlords, and other external entities.

• Coordinate and support a variety of recreational, social, and community-based activities.

• Encourage individuals to work toward their personal goals and increase independence.

• Provide person-centered support based on each individual's needs, interests, abilities, and preferences.

• Maintain appropriate documentation of services, activities, and participant progress.

• Communicate relevant concerns, progress, and changes to members of the individual's support team.

• Must possess and maintain a personal cell phone for work-related communication as a condition of employment.

Minimum Qualifications

Required:

• High school diploma or equivalent.

• Valid and clean New York State Driver's License .

• Experience as a Community Habilitation Specialist, babysitter, Home Health Aide, caregiver, or in a related support role.

• Own vehicle with valid proof of insurance, registration, and inspection.

• Ability to travel with participants in a vehicle for at least 30 minutes .

• Ability to work independently with remote supervision.

• Ability to identify community resources and activities that align with participants' interests and goals.

• Personable, approachable, patient, and dependable demeanor.

• Proficiency with computers and smartphones.

• Must possess and maintain a personal cell phone for work-related communication.

• Smartphone required.

Preferred:

• Previous experience working with individuals with intellectual and developmental disabilities (I/DD).

• Experience providing direct support, community habilitation, respite, home health, childcare, or other human services.

• Knowledge of community-based services and resources for individuals with developmental disabilities.

Work Location

Nassau and Suffolk Counties, NY

Community-based services may take place at various locations based on participant needs, goals, and service plans.

Personal vehicle required. Mileage reimbursement provided for approved work-related travel.

Work Hours / SchedulePort Washington

Full-time

Monday–Friday, 10:00 AM–4:00 PM

30-hour work week

Schedule includes holidays.

Other Program Sites

Part-time

Monday–Friday and/or Saturday–Sunday

3:00 PM–7:00 PM

Schedule includes holidays.

Specific schedules may vary based on participant and program needs.

Pay

$20.50 per hour

Part-time employees are also eligible for the 403(b) retirement plan and New York State Sick Leave.

Paid Time Off

• Up to 80 hours of vacation during the first calendar year of employment.

• Employees accrue an additional 8 vacation hours each subsequent calendar year , up to a maximum of 120 hours .

• Up to 40 hours of personal time per calendar year.

• 10 paid holidays.

Public Service Loan Forgiveness

As a non-profit organization, Community Mainstreaming Associates offers employees the opportunity to qualify for Public Service Loan Forgiveness (PSLF) , subject to applicable program eligibility requirements.
