Responsibilities
- Provide direct nursing care to residents within the licensed practical nurse scope of practice
- Monitor residents’ condition, perform routine assessments, and document findings
- Administer prescribed medications and treatments per physician orders and facility policy
- Support residents with activities of daily living and promote resident comfort and safety
- Document care and care plans accurately in the charting system and communicate changes to the care team
- Collaborate with registered nurses, physicians, and other disciplines to ensure coordinated, quality care
- Maintain infection control practices and safe patient handling, and respond promptly to changes in resident status

Requirements
- Active Licensed Practical Nurse license to practice (New Mexico) in good standing
- Completion of an accredited practical nursing program
- Ability to work 12-hour shifts in a skilled nursing facility environment
- Experience in long-term care or skilled nursing settings preferred
- Basic documentation skills and comfort with electronic health records
