Location: Lakeland / Davenport, FL and surrounding communities Compensation: $120,000-$125,000 Base Salary for Full-Time | Per Diem Rates Negotiable Additional Incentive: Sign-On Bonus Available for Full-Time Candidates About the Role Our client is a well-established, Medicare-certified home health organization providing skilled clinical and therapy services to patients throughout Florida. They are seeking a compassionate and motivated Physical Therapist (PT) to join their growing therapy team and provide in-home care throughout the Lakeland, Davenport, and surrounding Polk County communities . The organization has a strong geriatric focus, and this position will primarily work with older adults who need skilled therapy to improve mobility, strength, balance, safety, and overall independence within their homes. This is an excellent opportunity for a Physical Therapist who values one-on-one patient care and the autonomy of a field-based position while still having the support of an experienced interdisciplinary clinical team. Both Full-Time and Per Diem opportunities are available. Responsibilities Provide skilled physical therapy services to primarily geriatric patients in their homes throughout the assigned territory. Perform comprehensive physical therapy evaluations to assess mobility, strength, balance, gait, functional limitations, fall risk, and rehabilitation needs. Develop individualized plans of care based on each patient's clinical condition, functional goals, and home environment. Provide therapeutic interventions designed to improve strength, endurance, range of motion, balance, coordination, gait, and overall mobility. Provide gait training, transfer training, therapeutic exercise, and other evidence-based physical therapy interventions. Help patients regain or maintain the functional abilities necessary to safely remain as independent as possible in their homes. Evaluate patients for appropriate mobility and assistive devices and provide education regarding their safe use. Identify fall risks and other safety concerns within the patient's home environment and recommend appropriate interventions. Educate patients, family members, and caregivers on exercises, mobility techniques, fall prevention, positioning, and strategies to support continued progress between visits. Monitor patient progress and modify treatment plans based on clinical findings and established goals. Collaborate with physicians, nurses, occupational therapists, speech therapists, home health aides, and other members of the interdisciplinary care team. Communicate significant changes in patient condition, mobility, or functional status to the appropriate members of the care team. Complete clinical documentation accurately and within required timeframes. Maintain compliance with applicable Medicare, clinical, regulatory, and organizational standards. Qualifications Current Physical Therapist license in the State of Florida in good standing required. Graduate of an accredited Physical Therapy program. Valid driver's license and reliable transportation for travel throughout the assigned territory. Strong clinical assessment, communication, and documentation skills. Ability to independently manage a field-based patient caseload and schedule. Comfortable working with geriatric patients and individuals with varying levels of mobility and functional impairment. Strong patient and caregiver education skills. Previous home health experience is preferred but not required . Experience working with geriatric, rehabilitation, skilled nursing, orthopedic, or medically complex adult populations is preferred. Full-Time $120,000-$125,000 base salary , depending on experience. Support from an established clinical and administrative team. Per Diem opportunities are available for Physical Therapists seeking additional flexibility or supplemental clinical work. Per-visit rates are negotiable based on experience, availability, and territory coverage. Why This Opportunity? Home health experience is not required.
