Responsibilities Position is based in a skilled home health setting serving Medicare, Medicare Advantage, and commercial insurance patients Patients include med/surg, cardiac, orthopedic (hips, knees, shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, respiratory, neuro, trach, trauma, amputations, and ADLs Home visits required in the Davenport, Haines City, Lake Wales, and Winterhaven areas Typical caseload is 6-8 skilled home visits per day, with a minimum of 6 points per day, subject to adjustment by management Day shift schedule from 8:00 AM to 5:00 PM with self-scheduling based on office needs Required to work 1-2 weekends per contract Laptop, MiFi for internet, and portable printers provided for remote documentation and patient management Documentation and scheduling managed using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems Responsibilities include assessment, care planning, therapeutic interventions, education, and equipment evaluation in a home environment Duties include coordinating and documenting care, implementing home exercise programs, and instructing patients and families Team-based support with access to remote technology and equipment Solid color scrubs required for dress code First-time travelers will be considered Visits and work hours are self-scheduled for flexibility Requirements Minimum of 1 year recent home health physical therapy experience required Current state PT license required if applicable Valid driver’s license and reliable car for travel to patient homes Current BLS certification required OASIS training preferred Must be COVID vaccinated or provide approved exemption documentation Must have flu vaccination or submit declination with mandatory mask usage Experience with assessment, documentation, billing, equipment including gait belts, rolling walkers, sit-to-stand lifts, wheelchairs, mobility aids, and upper extremity prosthetics Competence in patient and family education, medication oversight, safety, and prevention of injury and abuse Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID #P-670957.
