Job Description Following direction of Registered Nurses and/or licensed physicians, provide patient care: perform skilled practical nursing procedures or treatments for clients in assigned work areas or in the clinic (insert catheters; reinstate gastrostomy tubes; irrigate ears and wounds; give enemas and douches; collect specimens such as urine, stool, or sputum; apply sterile dressing, ACE bandages, and elastic hose; apply topical ointments). Assist physicians in examinations, collect data (pulse, blood pressure, temperature, weight), and assist in emergencies (first aid, cardiopulmonary resuscitation, restraining clients as required). Administer immunizations and medications—controlled (narcotics, tranquilizers) and non‑controlled (aspirin, antibiotics, vitamins) in oral, rectal, or injectable forms—and provide instructions regarding medication goals, side effects, dosage changes, and more. Maintain medication accountability records consistent with Medicaid/Medicare, Department, and professional standards. Chart clients’ records, prepare and count medication using Physician’s Order Sheets, transcribe orders to Kardex/MAR, and record medication administration in clients’ charts. Observe and document clients’ conditions and responses to medication, treatment, behavior, and other significant activities. Complete episodic illness plans (OPFR) per procedure. Provide general nursing, direct care, and programming assistance: take vital signs; assist with personal grooming and hygiene, bathing; implement self‑administered medication programs; intervene in client behaviors such as aggression, agitation, or self‑injurious actions. Represent health services as a member of the interdisciplinary team, participate in meetings, and provide input regarding medications, behaviors, and health problems. Collect data for nursing quarterly assessments and annual IPP information for RN analysis and sign‑off. Participate in resident, interdisciplinary, special team, manager, monthly nursing, continuing education, and mandatory facility meetings. Provide training to direct‑care staff on health maintenance services. Prepare oral and written shift reports, coordinate with pharmacy regarding medication, and maintain equipment and supplies. In the absence of an RN at the workplace, may be assigned charge duty with supervising RN and/or physician on call. Qualifications License to practice nursing as a licensed practical nurse issued by the Ohio Board of Nursing pursuant to Section 4723 of Ohio Revised Code and MDS certification issued by ANNAC (American Association of Nurse Assessment Coordination).
