Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' mental health needs and develop personalized treatment plans.

• Diagnose and treat a spectrum of mental health disorders, modifying care plans as necessary to reflect changes in clients' statuses.

• Refer clients to relevant community services and resources, such as support groups, psychiatrists, or specialized treatment programs, to enhance their treatment outcomes.

• Collaborate with a multi-disciplinary team, including physicians and mental health professionals, to create and implement effective discharge plans and ensure continuity of care.

• Adhere to all federal, state, and local guidelines as well as clinical program standards, ensuring compliance with the Mental Health Practice Act.

• Provide education and support to clients and their families, fostering effective communication and advocacy throughout the treatment process.

• Capture important client information through our secure telehealth platform, maintaining a thorough and organized record of all therapeutic interventions.

• Assess for potential risks related to client care, including avoidance of discharges that may lead to readmissions or extended stays, and work with care providers to identify the best solutions.

• Offer debriefing and support sessions to staff, particularly in organizations connected to Employee Assistance Programs, following challenging scenarios to ensure team well-being.

Patient Population / Client Focus\n

Your primary focus will be delivering therapeutic support to diverse populations including individuals dealing with anxiety, depression, trauma, and relationship issues. You will engage with clients through secure video conferencing and phone consultations, ensuring that therapy is accessible regardless of the client’s geographic location. This role is crucial for promoting mental health awareness and adherence to treatment recommendations.

Qualifications\n

• Master's Degree in Social Work, Counseling, or a related field from an accredited institution.

• Current licensure as an LCSW, LMFT, LPC, or psychologist in Arizona.

• Demonstrated knowledge of community mental health resources.

• Technically proficient, with experience using telehealth platforms and electronic health records.

Compensation and Benefits\n
