PRP Worker - Rehabilitation Coordinator

New Day Health Systems in Catonsville, MD is looking for a passionate and dedicated PRP Worker - Rehabilitation Coordinator to join our team. In this rewarding role, you'll make a meaningful difference in the lives of individuals living with mental health challenges by helping them build skills, achieve goals, and live more independently. Responsibilities: * Provide Psychiatric Rehabilitation Program (PRP) services to individuals with mental health diagnoses in community and home settings
* Assist in developing and implementing individualized rehabilitation plans in collaboration with Rehab Specialist and clients and treatment teams
* Coach clients on daily living skills, social skills, and community integration strategies
* Monitor and document client progress toward rehabilitation goals in accordance with program requirements
* Coordinate with case managers, therapists, and other service providers to ensure continuity of care
* Transport and accompany clients to community-based activities and appointments as needed
* Maintain accurate, timely documentation and progress notes in compliance with Maryland COMAR regulations Requirements: * High school diploma or GED required; bachelor's degree in psychology, social work, or related field preferred
* Prior experience working in psychiatric rehabilitation, mental health, or human services
* Familiarity with Maryland PRP regulations and COMAR standards is a plus
* Strong interpersonal and communication skills with a client-centered approach
* Ability to work independently in community and home-based settings
* Valid driver's license and reliable transportation required
* CPR/First Aid certification or willingness to obtain upon hire About Us:
