Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a traveling PT - Home Health position serving patients across a range of medical and rehabilitative conditions in Sarasota and Charlotte County, FL.

Details

• Home health setting providing skilled visits to patients in their place of residence
• Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL patients
• Skilled home care services provided under Medicare, Medicare Advantage, and commercial insurance
• 6–8 skilled visits per day (minimum of 6 points per day, with potential increase at manager's discretion)
• Day shift schedule, 5x8 hours (8:00 AM – 5:00 PM)
• Weekend coverage required 1–2 weekends per contract
• Documents using BayCare Forms, BayCare Scheduling, and Ace Timesheet
• Equipment provided includes a laptop, MiFi for internet access, and a portable printer
• 20-week contract

Requirements

• Minimum 1 year of physical therapy experience required; home health experience required
• Current BLS certification required
• Valid state PT license required (license required if applicable)
• OASIS training preferred
• Proficiency in assessment, documentation, ortho, neuro, and cardiopulmonary/circulatory care
• Experience with home health equipment including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
• Competency in home exercise program development, patient/family education, medication management, safety and injury prevention, and billing
• Ability to recognize signs and symptoms of physical, sexual, and psychological abuse
• Valid driver's license and personal vehicle required to conduct home visits
• COVID vaccination required, or exemption form must be on file
• Flu shot required; candidates with a declination must wear a mask at all times while on assignment
