Lead Clinical Pharmacy Specialist

This position will be supporting the ICU at Holmes Regional Medical Center.

Position Summary

The Lead Clinical Pharmacy Specialist supports the planning, coordination and daily execution of clinical pharmacy services, including active participation in operational workflows, such as medication order processing, clinical consultation and verification, and pharmacy monitoring to ensure safe and efficient patient care delivery. The Lead Clinical Pharmacy Specialist facilitates the development, implementation, and maintenance of medication-use systems and evidence-based pharmacotherapy that promote optimal patient outcomes using safe and cost-effective drug therapies. Under the direction of pharmacy leadership, the Lead Clinical Pharmacy Specialist implements and evaluates goals and objectives and as such contributes to department growth and progressive pharmacy initiatives.

Primary Accountabilities

1. Provides day-to-day leadership and coordination of clinical pharmacy services, ensuring effective specialty coverage, team communication, and alignment with departmental objectives. Supports Health First's mission, strategic priorities, and organizational goals through excellence in clinical pharmacy practice, quality improvement, and interdisciplinary collaboration.

2. Leads the implementation and communication of approved medication-related guidelines, protocols, order sets, formulary changes, and educational initiatives with pharmacy and interdisciplinary stakeholders.

3. Promotes medication safety, quality, and financial stewardship through medication-use evaluations, formulary management, adverse drug reaction review, and clinical performance improvement activities.

4. Reviews, verifies, and validates medication orders and medication therapy regimens for appropriateness, safety, effectiveness, regulatory compliance, and optimal patient outcomes.

5. Delivers comprehensive pharmaceutical care through medication therapy management, evidence-based clinical interventions, and participation in interdisciplinary patient care activities. Serves as a clinical resource by providing drug information, medication education, and evidence-based therapeutic recommendations to providers, nurses, patients, and caregivers.

6. Collects, analyzes, and reports clinical, operational, stewardship, and pharmacoeconomic data to support performance improvement and demonstrate program outcomes.

7. Coordinates and optimizes clinical pharmacy services and operational workflows to support safe, efficient, compliant, and patient-centered care delivery.

8. Participates in the development, review, and implementation of pharmacy policies, procedures, therapeutic interchange programs, and non-formulary medication processes.

9. Partners with pharmacy leadership to design workflows, standardize clinical practices, coordinate staffing resources, and support integration of centralized and decentralized pharmacy services. Builds collaborative relationships across the organization through participation in committees, interdisciplinary initiatives, quality improvement activities, and professional presentations.

10. Evaluates and disseminates medical literature, clinical guidelines, and emerging best practices to support evidence-based medication use throughout the organization.

11. Serves as a preceptor, educator, and mentor while leading onboarding, competency development, clinical training, professional advancement, board certification support, and residency development initiatives for pharmacy staff.

12. Contributes to medication-use system operations, including medication preparation, dispensing, sterile compounding verification, inventory management, and medication distribution to ensure safe and compliant pharmacy services.

Work Experience

Minimum Qualifications

• Education: Bachelors degree in Pharmacy (BPharm).
• Work Experience: Any one of the following:
• Five (5) years years clinical pharmacy experience in direct pharmacotherapy decision making, direct patient care and pharmacy operations OR
• Completion of an ASHP accredited Post-Graduate-Year (PGY) 1 residency with two (2) years practice experience in an area of specialty.
• Licensure: Pharmacist licensure in the State of Florida.
• Certification:
• Any one of the following prior to start date and maintained:
• American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card OR
• American Heart Association Advanced Cardiac Life Support (AHA ACLS) AND Pediatric Advanced Life Support (AHA PALS) Healthcare Provider Completion Cards.
• Board of Pharmacy Specialist certification within one (1) year of start date and maintained.
• Skills/Knowledge/Abilities:
• Advanced knowledge of pharmacotherapy, disease state management and evidence-based medicine
• Ability to evaluate and apply medical literature to patient care
• Leadership and team coordination abilities
• Knowledge of formulary management, medication-use evaluations and quality improvement methodologies
• Strong communication and interpersonal skills
• Ability to multitask and manage competing priorities
• Ability to function effectively in fast-paced, high-acuity settings
• Flexibility as hours may be long and irregular; includes evenings, weekends and holidays as necessary

Preferred Qualifications

• Education: Doctorate of Pharmacy (PharmD).
• Work Experience:
• Completion of an ASHP accredited PGY 1 residency and PGY 2 residency in area of specialty.
• Formal or informal leadership experience including participation in pharmacy workgroups, specialty committees and round tables.
• Active preceptor to pharmacy students and residents, participation in research mentorship and program recruitment.
• Licensure: Consultant Pharmacist licensure in the State of Florida.
• Skills/Knowledge/Abilities:
• Ability to support clinical pharmacy services in any of the following areas: Medication Safety; Antimicrobial Stewardship; Oncology; Critical Care and Emergency Medicine; Pediatrics; Transitions of Care.
• Knowledge of project management and change management principles
• Knowledge of accreditation standards, state and federal regulations, and pharmacy practice standards
• Utilization of healthcare quality metrics, clinical outcomes measurement and data analytics

Physical Requirements

• Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
• May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
• May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
• May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
• May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
• May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
