As a Nurse Practitioner, youll engage with patients in their homes across Morris County, NJ, including areas such as Parsippany-Troy Hills, Mount Olive, and Randolph. These engagementseach lasting about 45 minutesmay be annual health assessments or post-discharge visits aimed at identifying healthcare needs and supporting quality outcomes for older adults. Schedule: Flexible shifts between Monday and Sunday Design your own schedule Minimum weekly or bi-weekly hourly commitment Responsibilities: Conduct comprehensive annual health risk assessments within patient homes Review medical history, current health status, and medication usage Educate members on conditions, treatments, and medications Detect care gaps and recommend follow-up or additional interventions as needed Document clinical findings accurately, without the need to create treatment plans or prescribe medications Qualifications: Active Nurse Practitioner License in New Jersey Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR certification (BLS) At least 6 months of experience as an NP (preferred) This per diem opportunity offers compensation of $80$90 per evaluation, plus mileage and a stipend for administrative tasks.
