Job Details Location: Miami Lakes, FL 33014 Job Title: Medical Social Worker (MSW) – PRN Employment Type: Per Diem (PRN) Schedule: Flexible, field-based Reports To: Director of Clinical Services / Clinical Manager Position Summary The Medical Social Worker (MSW) is responsible for providing social work services to patients in the home health setting. This role focuses on assessing psychosocial needs, connecting patients and families to community resources, and supporting overall care plan goals to improve patient outcomes and quality of life. Key Responsibilities Conduct initial and ongoing psychosocial assessments of patients and families Develop and implement individualized care plans in collaboration with the interdisciplinary team Provide emotional support and counseling to patients and caregivers Assist with community resource coordination, including financial, housing, and support services Support discharge planning and long‑term care planning Educate patients and families on available resources and coping strategies Maintain accurate and timely documentation in accordance with company and Medicare guidelines Participate in case conferences and care team meetings Qualifications Master’s Degree in Social Work (MSW) from an accredited program Minimum of 1 year of healthcare experience (home health experience) Strong knowledge of community resources and discharge planning Excellent communication and interpersonal skills Ability to work independently and manage time effectively Valid driver’s license and reliable transportation Additional Information Competitive per‑visit rates Flexible PRN scheduling based on your availability Opportunity to make a meaningful impact in your local community #J-18808-Ljbffr Senior Care Solutions Group
