Responsibilities:

Assess the social and emotional factors related to the client’s health status

Develop and implement a social service care plan

Collaborate with other healthcare professionals to ensure quality care

Maintain accurate and timely documentation of services

Provide counseling and emotional support to patients and families.

Facilitate support groups and educational sessions.

Connect patients with community resources and support services.

Collaborate with external agencies and organizations.

Advocate for patients' rights.

Participate in care conferences and collaborate with the healthcare team.

Maintain accurate and timely documentation of patient interactions

Requirements:

Licensed Medical Social Worker in the state of Florida

Excellent communication and interpersonal skills

Ability to work independently and as part of a team

Knowledge of medical terminology and healthcare regulations

Ability to work with a diverse population
