When you thrive, so does the community of care we're building together.Offer Based on Years of ExperienceBe the Best Hospice Registered Nurse/RN Team Director You Can BeIf you meet these qualifications, we would love to meet you:Associate degree preferredCertified Hospice Nurse (CHPN) preferredThree+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)Minimum of one year of supervisory experiencePossess and maintain valid CPR certification while employed in a clinical role (preferred)Must be a licensed driver who can travel to all business locationsMeet the regulations and requirements of the state(s) in which the program provides servicesResponsibilities:As a Hospice Registered Nurse Team Director, you will:Supervise direct patient and family care team members, in collaboration with program site leadershipResponsible for scheduling, payroll, and other administrative dutiesEnsure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronizationOversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdisciplinary plan of care for each patient and familyCollaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocolsParticipate and promote Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplinesCollaborate with site leadership to support staff education effortsFacilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality callsParticipate in or lead quality improvement activities, including data collection and aggregation as ableReinforces and supports all internal and external communicationsOur Investment in YouCaring for others starts with caring for you.
