Job Description
Title: Physical Therapist

Schedule: 8 hours per shift

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours per week

Requirements:
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license with reliable transportation and insurance - Required
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities:
- Perform clinical assessments to identify patient needs and appropriateness for therapy.
- Document and complete a Plan of Care (POC), including progress notes and other clinical record documentation.
- Plan and provide appropriate treatment for patients with neuromuscular, neurological, and orthopedic disorders.
- Instruct patients and families on safe transfer techniques and proper use of mobility equipment.
- Educate patients and families regarding therapeutic procedures based on individual needs and readiness.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care.
