Medicaid Home Care Assessments RN/LPN - PRN

Arch Home Care in Northern Virginia is seeking a qualified RN or LPN to conduct Medicaid home care assessments using DMAS forms on a PRN basis. The role focuses on evaluating medical, functional, and safety needs to support appropriate home care services.
You will complete required documentation, maintain confidentiality, and coordinate with clients, families, and providers. A Virginia license and CPR certification are required, with reliable transportation and strong communication skills
