Care Manager

CPC Integrated Health utilizes an integrated, client and family centered approach to treatment in all its programs. Treatment approaches are evidence-based and include CBT, MI, DBT. The case manager in the Adolescent program is part of an interdisciplinary team comprised of a dually licensed program supervisor, licensed clinician, and a second case manager, who is also certified in addictions (CADC). Adolescents aged 13-17 who have co-occurring mental health and substance use challenges will participate in a program that provides individual, group and family treatment along with wrap-around/case management services as needed. Youth will spend 2-4 months in the adolescent program before transferring to outpatient services for continued treatment, if needed. Title : Care Manager Location : Freehold, NJ MINIMUM QUALIFICATIONS: BA in a related field such as psychology, social work, etc. 2 years' experience working with youth and their families who are experiencing significant challenges in their lives. Basic understanding of mental health and substance use disorders in youth population. Basic understanding of evidence-based interventions such as Motivational Interviewing, DBT, CBT and harm reduction. Strong interpersonal, communication and organizational skills Strengths-based approach in working with youth and families Valid NJ driver's license and clean driving record. Basic computer skills and comfort with utilizing EHR Ability to engage youth in a positive non-judgmental manner Ability to manage multiple priorities and set priorities effectively Ability to remain calm in challenging situations PREFERRED QUALIFICATIONS: Master's degree in related field 5 years' experience working with youth Experience in a community setting with exposure to crisis management and de-escalation strategies. Proven skills in group facilitation. Experience delivering Motivational Interviewing (MI) with individuals across diverse populations and readiness levels Experience providing Cognitive Behavioral Therapy (CBT) interventions in individual and/or group settings Experience conducting universal screening for substance use disorders, including use of validated screening tools (e.g., SBIRT, AUDIT, DAST, or similar) MAJOR RESPONSIBILITIES/PERFORMANCE OBJECTIVES: Work with youth assigned to the adolescent program in group activities as assigned by clinician and/or supervisor. Develop supportive and strengths-based relationships with youth and their families and provide case management services as indicated in treatment plan. Coordinate services across CPC programs or with external providers such as schools, families, and other community partners involved with youth as directed by clinician and/or supervisor Monitor progress and document in accordance with all state and CPC policies. Complete all admission and discharge paperwork as instructed by clinician and/or supervisor. Support successful implementation of individualized treatment and safety plans for each youth in program Maintain appropriate professional boundaries while providing compassionate and respectful support Provide transportation to and from program for youth as assigned by clinician and/or supervisor Work closely with the team to support family involvement in treatment whenever possible Work closely with team to defuse and resolve crisis situations in a positive and constructive manner and to provide an environment that always supports safety of staff and youth Provide services using Motivational Interviewing (MI) and Cognitive Behavioral Therapy (CBT) consistent with fidelity standards Conduct universal screening for substance use disorders, ensuring accurate documentation and appropriate follow up Participate in required trainings and supervision related to MI, CBT, and other evidence based practices (EBPs) Demonstrate and maintain proficiency in EBPs through observation, skill demonstration, fidelity assessments or certification processes as required ESSENTIAL COMPETENCIES: Ability to work in a clinic setting, homes, and other non-clinical environments. Strong understanding of mental health, substance use, and social determinants of health. Excellent problem-solving and organizational skills. Ability to engage and build trusting relationships with diverse populations. Proficiency in electronic health records (EHR), Microsoft Office, and online resource navigation. Commitment to trauma-informed, person-centered, and culturally competent care. Flexibility in scheduling to meet client and program needs. An ability to independently travel to a variety of locations using a personal or agency-provided vehicle. WORK ENVIRONMENT & PHYSICAL REQUIREMENTS: This position will include field-based work, including home visits, community outreach, and travel between service locations. A valid driver's license, ability to drive, access to reliable transportation, including personal or agency-provided vehicle, and an acceptable driving record (per agency and insurance guidelines) are required. The role requires the ability to perform job-related tasks such as prolonged sitting, occasional walking, bending, stretching, and lifting up to thirty (30) pounds. It also requires sufficient coordination to operate standard office equipment and the ability to communicate and receive information, with or without reasonable accommodation.
