Registered Nurse Case Manager

Job Description

We are seeking a dedicated and Registered Nurse Case Manager to coordinate and oversee patient care across diverse healthcare settings. As a Registered Nurse Case Manager, you will collaborate closely with healthcare professionals to ensure optimal patient outcomes while adhering to health regulation policies and maintaining compliance with HIPAA standards.

Responsibilities

Completing the initial nursing assessment and developing an individualized plan of care.
Perform regular skilled nursing visits to assess the patient's condition and progress.
Supervising and directing License Practical Nurses, Registered Nurses and Certified Home Health Aides as required by state regulations and agency policy.
Manage utilization review processes by evaluating medical records, and treatment plans to ensure appropriate resource utilization.
Collaborate effectively with multidisciplinary teams including physicians, therapists, social workers, and other healthcare professionals to facilitate seamless patient care transitions.
Maintain accurate documentation while ensuring compliance with HIPAA regulations and medical terminology standards.
Participate in quality improvement initiatives.
