Actify Home Care is growing, and looking for Geriatric SLP home health therapists to join our team in Central Palm Beach County: West Palm Beach, Wellington, Royal Palm Beach, Riviera Beach, Lake Worth and surrounding areas. SLP Per-Visit Compensation (W-2 Employee): Evaluation / Re-Admission: $100.00 Routine Visit: $80.00 Why SLPs Choose Actify? What You’ll Do: Provide high-quality, one-on-one speech, cognitive, and swallowing therapy to geriatric patients in their homes (focusing on dysphagia, stroke recovery, and cognitive-linguistic care). We are open to skilled clinicians looking to transition their foundational SLP experience into adult home health. Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge). Develop and implement plans of care to address speech, language, and Dysphagia (swallowing) disorders. What You Need: License: Active Florida Speech-Language Pathologist (SLP) License.
