Join a dedicated healthcare team as an RN Care Manager, providing comprehensive coordinated care for adult patients across a regional healthcare system in the Midwest. This role involves developing, monitoring, and evaluating interdisciplinary care plans to ensure optimal patient outcomes.

Key Responsibilities:

• Conduct needs assessments and identify appropriate care options and services.
• Communicate effectively with patients, families, and interdisciplinary teams to support care coordination.
• Gather and manage patient data related to case management, care planning, and quality improvement analysis.
• Monitor care delivery through patient rounds, documentation, and progress tracking.
• Perform utilization reviews and assist nursing staff with patient care as needed.
• Support rehabilitation training and education for staff when required.
• Adhere to healthcare privacy regulations and organizational standards of care.
• Participate in process improvement initiatives and provide leadership on complex projects.

Education and Experience:

• Completion of an accredited nursing program.
• Minimum of one year experience as a registered nurse.
• Preferred: Bachelor of Science in Nursing (BSN) and care management experience.

Licensure and Certifications:

• Current RN licensure in the applicable state.
• Preferred: American Heart Association Basic Life Support (BLS) certification.
