Cross Country Nurses is seeking a travel nurse RN Home Health Hospice for a travel nursing job in Beaver Dam, Wisconsin. Job Description & Requirements Specialty: Hospice Discipline: RN Start Date: 09/21/2026 Duration: 6 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary: Home Health and Hospice Nurse in Beaver Dam, Wisconsin. This role involves providing compassionate care to primarily geriatric and occasional pediatric patients with a variety of diagnoses. The position emphasizes patient education, chronic disease management, and end-of-life care within a post-acute setting, supporting patients in their home environment. Job Details: Schedule: Monday through Friday, 8:00 am to 4:30 pm, with end times potentially varying. Weekend-only on-call shifts are required twice per week, Monday through Thursday. Required Qualifications: BLS (American Heart Association) certification; minimum of 1 year of clinical experience; active license to submit; location within 80 miles of the facility. Preferred Qualifications: Experience with home health or hospice care, chronic disease management, wound care, IV administration, and patient/family education. Float: No floating required; will perform both home health and hospice visits. Additional Information: The position includes a one-day computer-based orientation and 2-5 days of field training or shadowing depending on experience. Clinicians with prior employment with Marshfield must have an end date of at least six months before the submission date. Clinicians are required to clock in and out at the facility.
