This role involves providing outpatient therapy services within home health settings, supporting patient recovery and functional improvement across varied conditions. The position requires working closely with patients in their homes, offering compassionate and expert care in community-based environments. Job Details Position Description: Provides outpatient therapy services in home health settings, including treatments in homes, assisted living facilities, senior apartments, and board and care homes. Caseload involves 3 to 5 patients daily, with responsibilities including Start of Care visits, evaluations, and case management. The work encompasses varied diagnoses such as orthopedic, cardiovascular, pulmonary, oncology, neurological, geriatric, and amputations, excluding pediatrics and hospice. Required Qualifications: Valid Physical Therapist license, current BLS certification, valid drivers license, proof of automobile insurance, minimum of 2 years experience in a healthcare setting, including community or home health experience, and experience with EMR HomeCare Home Base (HCHB) preferred. Schedule: Monday through Friday, from 8:30 am to 5:00 pm, with a minimum of 40 hours per week. Additional Information: Need good oral and written communication skills, organizational skills, and ability to work in sometimes cramped or cluttered home environments. Solo practice is expected after a robust three-week orientation, which includes online learning and field precepting. Holiday work may be required; weekend work is optional. Scrubs are recommended, with a dress code of business professional including closed-toe shoes.
