Job Overview

This position involves the assessment and provision of physical therapy services in line with the standards set by our organization, as well as the laws and regulations applicable in Maryland, the District of Columbia, Virginia, and other relevant guidelines.

• Contribute to the department's goals and objectives while adhering to established policies, procedures, quality, and safety standards. Ensure compliance with governmental and accreditation regulations.
• Exhibit foundational leadership abilities as a Case Manager and fulfill additional responsibilities as needed.
• Act as the primary preceptor and mentor for newly hired nurses and home health aides, overseeing their field training. Collaborate with field trainers to ensure new hires gain the necessary clinical competencies and recommend further education based on identified skill gaps.
• Conduct Joint Observation Visits for skill validation and participate in mock surveys as required.
• Demonstrate a comprehensive understanding of Oasis documentation, ensuring accuracy and compliance with regulatory requirements, including care plan development and goal setting.
• Develop and implement effective physical therapy treatment plans in collaboration with patients, their families, physicians, and other healthcare team members, adjusting plans as necessary. Evaluations will include interviews, observations, palpation, and specialized assessments such as goniometry and muscle strength testing, utilizing appropriate tools.
• Independently execute and document care plans for patients with varying complexities to ensure continuity of care, reflecting the standard of care for each diagnosis and utilizing agency and community resources.
• Engage in service recovery efforts by addressing patient and customer concerns, coordinating with interdisciplinary teams, and implementing solutions that enhance patient satisfaction and operational excellence.
• Delegate cases to Physical Therapy Assistants (PTAs) as needed.
• Initiate and participate in regular case conferences, involving all relevant disciplines and including patients and families in goal setting and care plan adjustments.
• Maintain effective relationships with other departments and engage in multidisciplinary quality and service improvement initiatives. Represent the department in community outreach and participate in meetings and committees.
• Conduct patient history and assessments to identify habilitation and rehabilitation needs, recommend adaptive equipment or home modifications, and determine discharge planning requirements, including referrals to community resources.
• Perform various therapeutic procedures and modalities, including therapeutic exercises, functional mobility training, gait training, heat/cold applications, and ultrasound.
• Serve as an educator within the discipline, providing feedback to enhance workflow and best practices. Collaborate with the Education and Operations Team to create and update an individualized clinical orientation program for new staff.
• Carry out additional duties as assigned.

A Bachelor’s degree in Physical Therapy from an accredited institution is required.

1-2 years of clinical experience is necessary.

• PT - A valid Physical Therapist License to practice in the District of Columbia, Maryland, or Virginia, depending on the work location, is required upon hire.
• DL - A valid State Driver’s License in good standing and access to reliable transportation is required upon hire.
• CPR - Certification in Cardiac Pulmonary Resuscitation (including BLS and NRP) for healthcare providers from the American Heart Association (AHA) or American Red Cross is required within 90 days of hire.
• Additional unit or specialty certifications may vary by department.

• Strong clinical skills.
• Excellent organizational abilities.
• Outstanding verbal and written communication skills.
• Familiarity with home care equipment, reimbursement processes, and regulations governing PTAs.
• Basic computer proficiency is preferred.
• Experience with smartphones is beneficial.

The salary range for this position is USD $89,065.00 - USD $162,801.00 per year.
