This critical field role involves direct patient care and case management, aiming to enhance healthcare outcomes. - Salary: $82,500 - $97,800 Per Year, Depending on Experience

Responsibilities:

- Assess patient conditions and develop tailored care plans.

- Maintain detailed medical records and communicate patient status to relevant parties.

- Serve as the primary liaison among patients, their families, and healthcare providers.

- Educate patients and families on health management and preventive measures.

- Coordinate with healthcare professionals to ensure continuous care and efficient case management.

Requirements:

- License: Active RN license in Florida with no restrictions.

- Experience: At least 1 yea r in a similar in-home healthcare role.

- Skills: Proficient in case management software, exceptional organizational and communication skills.

- Traits: Empathetic, detail-oriented, and able to operate autonomously in field settings.
