Responsibilities

• Evaluate and treat patients with physical functioning disorders and wound care needs; develop and implement plan of care
• Administer interventions including manual techniques, ambulation training, therapeutic exercises, modalities, and use of assistive devices
• Assess patient progress, adjust treatments, and document outcomes in the clinical record per regulatory requirements
• Provide patient, family, and caregiver education and counseling related to physical disorders and rehabilitation goals
• Communicate progress, barriers, and treatment plans with the supervising clinician and health care team
• Supervise and support PT assistants, rehabilitation aides, and students; participate in required meetings and trainings
• Prepare comprehensive discharge summaries and ensure all required documentation is complete in the electronic health record
• Obtain necessary equipment and supplies per regulatory requirements; adhere to applicable practice acts and professional ethics

Requirements

• Missouri state license to practice Physical Therapy (active)
• Minimum 1 year of PT experience, LTC or Rehab setting preferred
• New graduates considered with Level II experience (not limited to Level I observation)
• Ability to work effectively in a long-term care/rehabilitation setting
• Proficiency with electronic documentation and regulatory reporting
• Commitment to professional standards: APTA guidelines and state practice acts
