Job TitlePhysician for primary care delivery to a panel of patients whose primary areas of responsibility will involve performance of and documentation of health care delivery for Veterans according to Veteran's Health Administration (VHA) and Durham Health Care System (HCS) standards.This physician will be fully qualified and certified to function in both a clinical and administrative role and in accordance with the guidelines and policies of the Durham Veterans Affairs Health Care System. This individual is delegated the authority and responsibility in his/her area of expertise to assume major clinical, consultative, educational, research, and administrative duties and assignments related to Veteran care. This is a dual clinical and administrative role.Functions of Scope Assigned Clinical:Primary Care Physician will be responsible for delivery of primary care to patients enrolled at the Durham VA HCS. The provider will manage scheduled and unscheduled patients in the outpatient clinic setting.Primary focus of practice is the independent clinical evaluation of Veterans including prevention and patient self-management. Comprehensive primary care will be provided to male and female Veterans.Care may be provided in many modalities including direct face to face care and virtual care (including telephonic, video, secure messaging, and others).Promotes, directs, evaluates, and manages clinical concerns that are found during these evaluations and is responsible requesting referrals, if consult services are needed.Functions as a member or in support of a Patient Aligned Care Team (PACT), collaborating with clinical and administrative support to provide timely, quality, coordinated care for Veterans.Utilizes and promotes approved standards of care, procedures, methods and techniques, safety precautions and protocols in their application to this population in all settings.Collaborates with other health care staff/resources to ensure continuity of care.Keeps abreast of latest trends, skills, procedures, and protocols relevant in the area of specialty and serves as a resource person for the medical staff.Primary providers in the outpatient setting are typically assigned a panel of patients to manage. Panel size at this time is determined by utilizing VHA software.Participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives.May need to assist in coverage for and care of the patients of colleagues. Provider may need to provide coverage in another Durham VA HCS primary care clinic. This would be within the same geographic area.Physicians need to follow departmental and VHA policies for leave and attendance and work within Title 38 appointment guidelines. Leave should be requested in as much advance as possible to minimize patient cancellations. All unscheduled absences must be properly communicated.Timely documentation and work completion is necessary in all areas in accordance with HCS policy.Quality of care should meet professional standardsIs direct supervisor for a subset of NP, PA and or physician at the primary care site.Functions as a on site primary care leader providing support and guidance on issues including but not limited to patient access, quality of care, educational initiatives, and efficient resource allocation.VA offers a comprehensive total rewards package.
