RN Case Manager: Discharge & Care Coordination
Inova Health
Inova Alexandria Hospital is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team. This Full-Time role covers Monday–Friday with rotating weekends and emphasizes discharge planning and continuity of care for acute and post-acute settings. The successful candidate will coordinate services, liaise with patients and families, and apply utilization management to determine medical necessity and efficiency.
