RN Case Manager: Care Coordination & Discharge Planning

MedStar Health’s Washington Hospital Center is seeking a Registered Nurse in Case Management to apply clinical expertise to care coordination and discharge planning across the patient care continuum. The role collaborates with a multidisciplinary team to design individualized care plans, ensure quality and cost-effective outcomes, and manage resources during acute hospitalization and post-discharge phases. Requires ADN; BSN preferred, 2–3 years clinical experience and DC/MD licensure; CCM #J-18808-Ljbffr
