Responsibilities
- Perform initial and ongoing physical therapy assessments for residents in the skilled care setting
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional tasks
- Provide interventions such as gait training, therapeutic exercises, balance retraining, and functional mobility activities
- Monitor progress, document outcomes, and adjust treatment plans as needed
- Collaborate with the interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to optimize care and discharge planning
- Maintain accurate, compliant documentation in medical records and adhere to regulatory standards

Requirements
- Active Physical Therapist license in New Mexico or eligibility to obtain licensure
- Doctor of Physical Therapy (DPT) degree or equivalent
- Basic Life Support (BLS) certification
- Ability to work in a skilled nursing facility environment and manage a patient caseload
- Proficiency with electronic health records and clinical documentation
- Eligibility for travel assignments within the United States
